A list of your suspected food allergies and the symptoms you are experiencing with them.

What are you looking from the allergist? There is no cure for food allergies, but they can prescribe an Epi-Pen if your symptoms are severe enough. Otherwise they typically recommend Benadryl similar anti-hystamines which are over the counter.

If you are experiencing skin problems due to your food allergies they might recommend seeing a Dermatologist.

Otherwise just bring what you would to any normal doctor's appointment like your entire medical history so you can fill in the questionnaire at the beginning without struggling to remember when you are there. I have my most important information written out on several sheets.
